Requirements
- Active RN license
- 1+ year of clinical nursing experience
- Strong assessment and critical thinking skills
- Dependable transportation and valid driver’s license
- Ability to work independently and manage your day
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
Responsibilities
- Provide direct nursing care in the home setting
- Assess patient condition and make clinical decisions
- Follow and implement the plan of care
- Monitor for changes and report to care team
- Document visits and patient updates in a timely manner
- Collaborate with RN Case Manager and leadership
- Maintain clear communication with patients and families
