Requirements

• Active RN license
• 1+ year of clinical nursing experience
• Strong assessment and critical thinking skills
• Dependable transportation and valid driver’s license
• Ability to work independently and manage your day
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Responsibilities

• Provide direct nursing care in the home setting
• Assess patient condition and make clinical decisions
• Follow and implement the plan of care
• Monitor for changes and report to care team
• Document visits and patient updates in a timely manner
• Collaborate with RN Case Manager and leadership
• Maintain clear communication with patients and families
