About The Position

Aviata Health Group is seeking a Weekend Nurse Practitioner (Skilled Nursing) to serve as the leading provider of internal medicine services on Saturdays and Sundays at Bryan Dairy. In this role, you will collaborate with the facility's interdisciplinary team and physicians to reduce hospitalizations on weekends. You will work with the entire facility's patient population, including short-term and long-term residents, with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physicians. The position involves evaluating new admissions, coordinating discharge visits, and assessing patients exhibiting changes in condition. As a SNF-ist, you will provide advanced care planning, comprehensive health assessments, differential diagnoses, and prescribe treatments under protocol for acute and chronic illnesses. You will also prescribe medications, perform physical examinations, develop patient management policies, and facilitate referrals.

Requirements

  • Advanced Practice Licensure (APN) with a Florida license
  • Must have an active license and DEA number.
  • Previous experience in nursing or other medical fields.
  • Ability to build rapport with patients and their loved ones.
  • Ability to thrive in a fast-paced environment.
  • Excellent written and verbal communication skills.
  • Strong leadership experience with emphasis on teamwork in a healthcare setting.
  • Master’s degree required.

Responsibilities

  • Act as a Skilled Nursing Facility’s leading provider of internal medicine services on Saturday and Sunday.
  • Collaborate with the facility’s interdisciplinary team and physicians to reduce hospitalizations on weekends.
  • Work with the entire facility’s patient population - short and long-term residents.

  • Prescribe all necessary medications and treatments.
  • Perform comprehensive physical examinations of patients.
  • Develop and implement patient management policies and procedures.
  • Facilitate referrals to other healthcare professionals and medical facilities.
  • Communicate with attending physicians and/or specialists regarding patient care.
