About The Position

The Weekend Nurse Practitioner (Skilled Nursing) will act as the Skilled Nursing Facility’s leading provider of internal medicine services on Saturday and Sunday. The role involves collaborating with the facility’s interdisciplinary team and physicians to reduce hospitalizations on weekends. The Nurse Practitioner will work with the entire facility’s patient population, including short and long-term residents, with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians. Key responsibilities include evaluation of new-admission visits, coordination of discharge visits, and evaluations for patients exhibiting changes in condition. The role also involves providing advanced care planning, comprehensive health assessments, differential diagnosis, prescribing treatments, medications, performing physical examinations, developing patient management policies, and facilitating referrals.

Requirements

  • Advanced Practice Licensure (APN) with a Florida license
  • Must have an active license and DEA number.
  • Previous experience in nursing or other medical fields.
  • Ability to build rapport with patients and their loved ones.
  • Ability to thrive in a fast-paced environment.
  • Excellent written and verbal communication skills.
  • Strong leadership experience with emphasis on teamwork in a healthcare setting.
  • Master’s degree required.

Responsibilities

  • Act as the Skilled Nursing Facility’s leading provider of internal medicine services on Saturday and Sunday.
  • Collaborate with the facility’s interdisciplinary team and physicians to reduce hospitalizations on weekends.
  • Work with the entire facility’s patient population - short and long-term residents.
  • Provide evaluation of new-admission visits.

  • Provide Advanced Care Planning.
  • Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
  • Prescribe all necessary medications and treatments.
  • Perform comprehensive physical examinations of patients.
  • Develop and implement patient management policies and procedures.
  • Facilitate referrals to other healthcare professionals and medical facilities.
  • Communicate with attending physicians and/or specialists regarding patient care.
