About The Position

Aviata Health Group is seeking a Weekend Nurse Practitioner (Skilled Nursing) to serve as the leading provider of internal medicine services on Saturdays and Sundays at their St. Cloud, Florida location. In this role, you will collaborate with the facility's interdisciplinary team and physicians to reduce hospitalizations on weekends. You will manage the care of both short-term and long-term residents, with support from the facility's nursing staff and guidance from medical directors and attending physicians. Responsibilities include evaluating new admissions, coordinating discharge visits, and assessing patients with changes in condition. The role involves providing advanced care planning, comprehensive health assessments, differential diagnoses, and prescribing treatments for acute and chronic illnesses. You will also prescribe medications, perform physical examinations, develop patient management policies, and facilitate referrals to other healthcare professionals.

Requirements

  • Advanced Practice Licensure (APN) with a Florida license.
  • Must have an active license and DEA number.
  • Previous experience in nursing or other medical fields.
  • Ability to build rapport with patients and their loved ones.
  • Ability to thrive in a fast-paced environment.
  • Excellent written and verbal communication skills.
  • Strong leadership experience with emphasis on teamwork in a healthcare setting.
  • Master’s degree required.

Responsibilities

  • Act as a Skilled Nursing Facility’s leading provider of internal medicine services on Saturday and Sunday.
  • Collaborate with the facility’s interdisciplinary team and physicians to reduce hospitalizations on weekends.
  • Work with the entire facility’s patient population - short and long-term residents with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.
  • Evaluation of new-admission visits.
  • Coordination of discharge visits.
  • Evaluations for patients exhibiting changes in condition.
  • Provide advanced care planning.
  • Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
  • Prescribe all necessary medications and treatments.
  • Perform comprehensive physical examinations of patients.
  • Develop and implement patient management policies and procedures.
  • Facilitate referrals to other healthcare professionals and medical facilities.
  • Communicate with attending physicians and/or specialists regarding patient care.
