About The Position

We are seeking an experienced and compassionate Part-time Weekend Registered Nurse (RN) Case Manager for our Hospice division in the Snohomish County area. This role involves delivering complex, one-on-one care to patients in their homes, focusing on a wide range of medical needs. The position offers autonomy and flexibility to manage your own schedule, fostering critical thinking and specialized skills. You will work alongside top-tier clinicians and office staff, backed by accessible leadership that values your input, in a fun and mission-driven culture.

Requirements

• Active Washington State Registered Nurse (RN) license.
• Current BLS certification from the American Heart Association.
• Valid driver's license and insured, reliable vehicle.

Nice To Haves

• Prior Home Health and/or Hospice experience is strongly preferred.

Responsibilities

• Provide skilled nursing care and symptom management to terminally ill patients.
• Develop and manage individualized care plans in coordination with the hospice team.
• Educate and support patients and families through end-of-life care.
• Administer medications and treatments for comfort and quality of life.
• Coordinate care with physicians, social workers, aides, and other team members.
• Respond to urgent needs and provide crisis support when necessary.
• Maintain accurate and timely documentation in the EMR.
• Ensure compliance with hospice regulations and standards.
