About The Position

Home Health Care, Inc. is seeking experienced Registered Nurses (RNs) to join their weekend admissions team. This role focuses on performing Start of Care visits for new home health patients. The position offers a unique compensation structure of $200 per Start of Care visit plus mileage reimbursement, with weekend-only scheduling. It is ideal for experienced home health nurses who want to supplement their income, pick up additional work on their terms, or prefer focusing on admissions rather than ongoing case management. The role involves assessing patients, establishing care plans, and coordinating services to ensure patients begin their home health journey on the right path.

Requirements

• Active Minnesota RN license.
• Current CPR certification.
• Home health nursing experience required.
• OASIS experience required.
• Strong clinical assessment and documentation abilities.
• Confidence making independent clinical decisions in the field.
• Valid driver's license, active insurance, and reliable transportation.

Nice To Haves

• Start of Care or admission experience strongly preferred.
• Military medical experience is highly valued.
• Applicable military codes include Army 66H, 66N, 66R; Navy 290X, 1960; Air Force 46AX, 46FX; Marines MED; and Coast Guard HS.

Responsibilities

• Complete Start of Care admissions for new home health patients.
• Perform comprehensive skilled nursing assessments.
• Complete accurate and timely OASIS documentation.
• Review medications, diagnoses, safety concerns, and disease-management needs.
• Establish individualized plans of care.
• Educate patients and caregivers on their care at home.
• Identify additional nursing, therapy, social work, or other service needs.
• Coordinate with physicians and interdisciplinary clinicians.
• Maintain accurate and compliant electronic documentation.
