About The Position

Concierge Home Care is seeking a Weekend Home Health Registered Nurse (RN) in Clearwater, FL. This role offers a dedicated weekend schedule, allowing nurses to keep their weekdays free while providing essential skilled nursing care to patients in their homes. The position emphasizes flexibility, supportive leadership, and modern technology, including AI-assisted charting tools, to enhance efficiency and patient focus. Nurses can expect one-on-one patient care, away from the demands of hospital rotations, with a pay-per-visit model designed for motivated clinicians.

Requirements

• Active, unencumbered Florida RN license.
• Valid driver’s license, reliable transportation, and auto insurance.
• Must be able to complete an AHCA Level 2 background screening.

Nice To Haves

• Home health experience preferred, but we are happy to train the right candidate!

Responsibilities

• Conducting patient assessments and developing individualized care plans.
• Providing skilled nursing care and essential patient/caregiver education.
• Coordinating care with physicians and interdisciplinary teams.
• Monitoring progress and adjusting care for patients recovering at home.
• Documenting visits efficiently using our streamlined AI-assisted tools.
