About The Position

As a Home Health Registered Nurse, you’ll deliver skilled nursing care in patients’ homes, creating individualized care plans, administering medications, educating patients and families, and supporting recovery and independence. The role offers flexibility, autonomy, and meaningful one-to-one patient relationships for nurses experienced in home health or transitioning from hospital, acute care, or skilled nursing settings.

This full-time position is based with Trinity Health At Home – Muskegon in Michigan and follows a Thursday–Saturday schedule of three 12-hour shifts. It offers predictable assignments, nurse-led support, career growth opportunities, and use of the Epic EHR system for documentation and care coordination. There are no on-call requirements.

Requirements

• Graduate of an approved nursing education program.
• Current Registered Nurse licensure in the State of Michigan.
• At least one year of professional registered nursing experience.
• Strong computer and technology skills.
• Current driver’s license and reliable transportation.

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred.
• Medical-surgical experience preferred.
• Home health or hospice experience is a plus.
