About The Position

Pyramid Healthcare is seeking a passionate and community-focused Vocational Specialist (Qualified Professional) to join our Assertive Community Treatment Team (ACTT) in Winston-Salem, NC. This rewarding community-based role is ideal for someone dedicated to helping individuals with severe mental illness build independence through employment support, housing stability, life skills development, and recovery-focused services. Our ACT Team operates as a “hospital without walls,” delivering intensive behavioral health services directly in the community. We meet individuals where they are — at home, work, or in the community — helping remove barriers and create meaningful, lasting progress. The Vocational Specialist (QP) assists individuals served by the ACT Team in obtaining and maintaining competitive employment while supporting overall recovery, independence, and community integration. This role collaborates closely with a multidisciplinary treatment team to provide person-centered, strengths-based services.

Requirements

  • Bachelor’s degree in Social Work, Counseling, Psychology, Human Services, or related field
  • Must meet Qualified Professional (QP) status under NC regulations (10A NCAC 27G.0104)
  • Valid driver’s license with acceptable driving record
  • Ability to work in the community and transport clients as needed

Nice To Haves

  • Experience in vocational rehabilitation, supported employment, ACTT, housing services, or mental health
  • Experience working with individuals with severe and persistent mental illness
  • Master’s degree in a Human Services field

Responsibilities

  • Assist individuals in identifying career interests, strengths, and employment goals
  • Support job readiness, resume development, interview preparation, and applications
  • Develop relationships with local employers and community partners
  • Provide job coaching and ongoing retention support after placement
  • Promote competitive employment opportunities aligned with recovery goals
  • Assist clients with obtaining and maintaining stable housing
  • Connect individuals with transportation, benefits, and community resources
  • Support independent living skills, budgeting, and daily functioning
  • Participate in treatment planning and interdisciplinary team meetings
  • Provide progress updates and recommendations to the treatment team
  • Coordinate care with families, referral sources, and community providers
  • Complete timely and accurate documentation of services provided
  • Maintain compliance with ACTT, Medicaid, CARF, and state regulations
  • Participate in quality improvement initiatives and peer record reviews
  • Provide transportation for clients when needed
  • Participate in rotating crisis response and emergency coverage as assigned
  • Assist with hospital diversion and community stabilization efforts
