About The Position

Providence Care, which has merged with Your Health, is hiring full-time Registered Nurses to provide primary care services to patients in their homes, assisted living facilities, and long-term care facilities. Patients are enrolled in the organization’s primary care program. The role includes case management, oversight of patient care plans, supervisory visits, start-of-care visits, and education for patients, families, and facility staff.

Requirements

• Hold an unencumbered RN license in the state where applying.
• Have reliable transportation and a valid driver’s license.
• Have a clean motor vehicle record (MVR).

Nice To Haves

• Experience in med/surg, hospice/home health, or primary care.

Responsibilities

• Provide case management to patients and families, including symptom management, medication management, and collaboration with members of the IDG.
• Oversee patient plans of care in collaboration with the collaborating physician.
• Provide supervisory visits and oversight for LPNs and CNAs.
• Conduct start-of-care visits and collaborate with Care Navigators.
• Provide education to patients, families, and facility staff.
