About The Position

Looking for an RN role that offers more independence, one-on-one patient care and the ability to manage your day outside of a traditional hospital setting? Join ProHealth Home Care & Hospice as a Visit RN and provide skilled nursing care to Home Health and Hospice patients throughout our community. You'll have the autonomy of field nursing with the support of an experienced interdisciplinary ProHealth Care team behind you.

Requirements

• Current Wisconsin RN license
• Current BLS certification
• Valid driver's license and acceptable driving record
• Strong assessment, critical-thinking and communication skills
• Ability to independently prioritize patient needs and manage a field-based schedule
• Excellent time management skills

Nice To Haves

• Approximately two years of RN experience preferred
• Home Health experience preferred
• Experience in acute care, medical-surgical nursing, wound or ostomy care, IV therapy, infusion, oncology, rehabilitation or other hands-on clinical settings may translate well to this role.
• Strong assessment skills, clinical judgment, organization, adaptability and confidence working independently

Responsibilities

• Nursing assessments and monitoring for changes in condition
• Wound and ostomy care
• Medication education and management
• Injections and IV therapy
• Patient and caregiver education
• Chronic and complex disease management
• Comfort and symptom management for Hospice patients
• Communication with physicians and other members of the care team
• Coordination with nurses, therapists, social workers and other disciplines
• Documentation in Epic
