About The Position

The Virtual Care Center provides rapid access to same-day telehealth appointments for patients with acute medical conditions. The Registered Nurse supports the prioritization of acute patients, helps ensure they are seen in a timely manner, and monitors provider schedules to facilitate patient flow. The RN plays an important role in ensuring patients receive quality care through telemedicine. This is a remote, work-from-home position, with onsite work at Bronx locations as needed.

Requirements

• Bachelor's degree in Nursing.
• New York State Licensed Registered Professional Nurse.
• 1–2 years of direct clinical experience in a primary care environment working with adults and children.
• Bilingual English/Spanish proficiency.
• Previous experience or expertise using electronic health records.
• Ability to thrive in a telemedicine environment.

Responsibilities

• Triage patients based on acuity and ensure they are seen in a timely manner.
• Monitor the call center template for pediatric and adult patients and discharged patients.
• Manage abnormal laboratory and diagnostic imaging results and critical values.
• Answer clinical calls from hospitals, pharmacies, the Department of Health, call center agents, and other settings.
• Follow up with remote patient monitoring (RPM) patients and assess acuity based on their vital signs.
• Provide clinical guidance and support to Virtual Care Center Medical Assistants.
• Coordinate patient flow to providers throughout the organization, prioritizing patients based on acuity.
• Perform other duties as assigned by the Director of Virtual Care Center.
