About The Position

Devoted Medical is building a virtual-first, value-based chronic kidney disease clinic to expand specialty care access for vulnerable seniors. The clinic aims to delay progression of CKD stages 4 and 5 and increase planned, rather than emergency, dialysis starts. It is one of Devoted Medical’s virtual specialty care programs, designed to provide tailored care for patients with specific chronic conditions. The Advanced Practice Provider delivers direct nephrology care to patients with chronic kidney disease, including medication optimization, electrolyte and volume management, shared decision-making about goals of care, and planning for vascular access or peritoneal dialysis catheter placement. The role collaborates with the nephrology care team, patients’ families, and caregivers to coordinate holistic, continuous care. It also includes panel reviews, comprehensive clinical and social needs assessments, use and feedback on the organization’s electronic health information system, accurate recordkeeping, quality improvement, ongoing professional development, and compliance with applicable laws, privacy requirements, and telehealth regulations.

Requirements

Nurse Practitioner or Physician Assistant with at least 5 years of outpatient clinical practice experience.
At least 2 years of experience in a nephrology practice or dialysis unit.
Current, active, unrestricted NP or PA license in at least one U.S. state.
Willingness to obtain additional state licenses as required by the business.
Active BLS certification at hire and maintained throughout employment.
Thorough understanding of nephrology management, including disease processes, treatment modalities, medication management, and lifestyle modification.
Direct experience working in a nephrology practice and/or dialysis center.
Experience working on an interprofessional team.
Proficiency with telehealth technology and electronic health records.
Strong clinical and non-clinical judgment, thoroughness, empathy, and commitment to patient-centered care.

Nice To Haves

Virtual care experience and a strong desire to continue practicing clinical medicine through virtual visits.
Understanding of managed care, including assessing STARS/HEDIS measures, coding clinical comorbidities, and identifying clinical care gaps.
Proficiency in Spanish.

Responsibilities

Provide direct nephrology care to patients with chronic kidney disease, including medication optimization and management of electrolytes and volume status.
Facilitate shared decision-making about goals of care and plan for vascular access or peritoneal dialysis catheter placement to support optimal dialysis initiation.
Collaborate with nephrology physicians, social workers, pharmacists, clinical assistants, family members, and caregivers to coordinate holistic care and continuity.
Participate in regular panel reviews and provide guidance on medical management.
Perform comprehensive assessments, including diagnosis and disease review, medication review, quality-of-care interventions such as STARS/HEDIS, and assessment of social, home health, and durable medical equipment needs.
Use the organization’s electronic health information system for visits and provide feedback to improve its interface.
Maintain accurate, up-to-date patient records in compliance with legal and ethical guidelines.
Participate in quality improvement initiatives and ongoing professional development to stay current with kidney-care practices and advancements.
Follow applicable laws, regulations, industry standards, patient privacy requirements, and telehealth regulations.
