About The Position

The Violence Intervention Specialist will be the primary frontline team member, who will introduce the HVIP and its services bedside. They will assess patient risk and immediate needs. They will provide a warm hand-off to the HVIP's LCSW therapist for mental healthcare and determine eligibility for the Indy Peace Fellowship. When patients are eligible for the Indy Peace Fellowship, the Violence Intervention Specialist will receive consent and refer into it. When patient's do not meet eligibility criteria for the Indy Peace Fellowship, when they reside outside of Marion County, or when case management needs arise, the Violence Intervention Specialist will be the intensive case management provider. Serves as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up.

Requirements

  • High School Diploma or equivalent required.
  • 0-3 years of experience required.
  • Requires basic computer skills.
  • Requires knowledge of medical, community, and social resources.
