The Case Manager participates in outreach activities to identify and enroll SSVF-eligible veterans in case management services, ensuring timely access to safe, permanent housing and essential supportive services. The position utilizes a Housing First Model to develop and implement Housing Stability Plans that promote household stability and self-sufficiency. Conduct full assessments with applicants to enroll SSVF-eligible veterans in case management for a minimum of 90 days, documenting all activities and events Ensure rapid entrance into permanent safe housing or maintain stable housing to prevent homelessness Mediate, advocate, and leverage support for eligible veterans, referring them to healthcare navigation, legal, employment, financial, and other services as needed Identify and connect veterans to community resources for overall well-being, including emergency housing needs Maintain accurate records of all resources provided, including temporary financial assistance, and manage client files safely while preserving confidentiality Represent Baker Ripley and the Veteran Services team professionally in internal and external meetings and events Collaborate with other agencies to provide a comprehensive case management process Perform other duties as assigned
