About The Position

United Way Miami, Inc. is hiring a Veterans Case Manager, MU to join their team. This role provides holistic, client-centered case management with service and resource coordination, empowering veterans, transitioning service members, and military families to advance toward economic mobility and stability. Requirements

  • Bachelor’s degree with major course work in social work, psychology or other field closely related or equivalent experience
  • At least 2 years of experience in social case work, counseling, eligibility determination, social service or community resource referral, or closely related work.
  • Intermediate Microsoft Office suite, including Outlook, and knowledge of modern web browsers.
  • Must be detail-oriented and able to multitask in a fast-paced environment.
  • Strong customer service skills required.
  • Ability to function in a team environment.

Nice To Haves

  • Military experience/Veteran highly preferred.
  • Case management background preferred.
  • Knowledge of the needs of the Military/Veterans is essential.
  • Spanish language not a requirement, but helpful.

Responsibilities

  • Performs needs and comprehensive assessments; develops or updates care plans, service plans, and/or treatment plans with clients and their families to address their needs and to facilitate referral to appropriate treatment or service providers or facilities; engages clients and families in the development of service implementation plans
  • Counsels clients and/or sustaining success and wellness; conducts activities and monitors the service environment in a manner to maximize client success and well-being
  • Advocates on behalf of clients and families for services, basic needs, and other related issues
  • Composes or prepares correspondence, case notes, narrative and technical reports, notifications, and related documents using computer-based applications
  • Perform comprehensive, client-centered social work activities in accordance with best practices
  • Conducts community and home visits (if needed) related to client needs; provides follow-up and monitors service delivery and care environments
  • Stay abreast of community resources and procedures for referring clients to appropriate resources
  • Facilitates case consultation for client situations
  • Provides crisis prevention, intervention, and supportive counseling as needed.
  • Performs other duties and assumes other responsibilities as assigned by the supervisor
  • Fully engaged in the organization's crisis plan and response efforts.
