About The Position

The Veterans Affairs (VA) Specialist is responsible for coordinating and managing services for veteran clients who receive in-home care through the U.S. Department of Veterans Affairs and related veteran benefit programs. This position serves as a primary resource for veterans, their families, caregivers, VA representatives, referral partners, and the agency's care team. The VA Specialist will assist with identifying eligible veterans, coordinating referrals, navigating VA programs and requirements, maintaining accurate documentation, and supporting the authorization and continuation of in-home care services. The ideal candidate is knowledgeable about VA benefits, highly organized, compassionate, and comfortable communicating with veterans and their families.

Requirements

  • High school diploma or equivalent.
  • Experience working with veterans, VA programs, healthcare, home care, case management, insurance authorization, or a related field.
  • Strong organizational and documentation skills.
  • Excellent verbal and written communication skills.
  • Ability to work professionally with veterans, families, healthcare providers, and government representatives.
  • Ability to maintain confidential and sensitive client information.
  • Strong attention to detail and ability to manage multiple cases, deadlines, and follow-ups.
  • Proficiency with Microsoft Office and electronic client-management or healthcare systems.

Nice To Haves

  • Associate's or bachelor's degree in healthcare administration, social services, business administration, human services, or a related field.
  • Previous experience working directly with the Department of Veterans Affairs or VA-funded home care programs.
  • Knowledge of VA Community Care programs and processes.
  • Experience with home health, personal care, homemaker/home health aide, or other in-home care services.
  • Experience with VA referrals, authorizations, billing, or Community Care Network processes.
  • Experience working with electronic health records or home care management software.

Responsibilities

  • Serve as the agency's primary point of contact for VA-related client services and referrals.
  • Educate veterans and their families about available VA home care benefits and services.
  • Assist prospective and existing veteran clients with navigating VA programs, eligibility requirements, referrals, and authorizations.
  • Coordinate with VA Medical Centers, Community Care Network representatives, VA social workers, case managers, and other authorized VA personnel.
  • Process and track VA referrals, authorizations, service requests, renewals, and changes in care.
  • Monitor authorization dates and ensure services remain properly authorized and documented.
  • Maintain accurate and up-to-date veteran client records in accordance with agency, VA, and applicable regulatory requirements.
  • Communicate authorization changes and client needs to caregivers, scheduling staff, clinical personnel, and agency leadership.
  • Assist with resolving authorization, billing, scheduling, documentation, and service-related issues involving VA-funded clients.
  • Follow up with referral sources and VA representatives regarding pending referrals and service approvals.
  • Coordinate intake and onboarding of new veteran clients.
  • Help ensure that services provided match the authorized plan of care and applicable program requirements.
  • Maintain confidentiality of veteran and client information.
  • Prepare reports and maintain tracking systems for VA referrals, authorizations, utilization, and renewals.
  • Monitor changes in VA policies, procedures, programs, and requirements that may affect the agency or its veteran clients.
  • Build and maintain positive relationships with veteran organizations, VA facilities, healthcare professionals, social workers, discharge planners, and community referral partners.
  • Participate in community outreach and educational activities designed to increase awareness of the agency's services among veterans and their families.
  • Perform other duties as assigned by management.
