About The Position

The YWCA Greater Harrisburg is seeking a passionate and dedicated full-time Case Manager to join our team as a Transition In Place (TIP) Veteran Case Manager position. In this rewarding role, you'll assist homeless Veteran households to identify, obtain, and maintain permanent housing to achieve their housing stability and self-sufficiency goals through provision of community outreach, case management, development and monitoring of Individual Service Plans (ISP), coordination of service provision, instruction of life skills, provision of supportive services, and development of collaboration with existing and potential housing providers. Requirements

  • Bachelor's degree in human services or related field with prior related work experience is preferred. An equivalent combination of education and experience will also be considered.
  • In-depth knowledge and understanding in social services, veteran benefits, and other supportive services.
  • Valid driver's license, clean driving history, and reliable personal vehicle. Proof of insurability may also be required.
  • Current FBI, criminal, and child abuse clearance or the ability and willingness to obtain.

Responsibilities

  • Conduct outreach to individuals experiencing homelessness and assess eligibility for program participation.
  • Build partnerships with community providers to coordinate supportive services.
  • Complete intake, enrollment, and eligibility determinations for program applicants.
  • Provide intensive case management using the Critical Time Intervention (CTI) model, including ISP development, goal tracking, and progress monitoring.
  • Ensure participant safety and access to shelter, basic needs, and stable housing.
  • Deliver supportive services such as budgeting, life skills, transportation coordination, and housing stabilization support.
  • Educate participants on program expectations, rights, and compliance requirements.
  • Support housing retention by collaborating with landlords and mediating housing-related concerns.
  • Assist participants with exit planning and linkage to ongoing community resources.
  • Maintain timely and accurate documentation and required program reports.
  • Participate in outreach events and support program, department, and organizational initiatives.
