The Vascular Nurse Practitioner (NP) is responsible for providing comprehensive mobile vascular care to patients residing in skilled nursing facilities, rehabilitation centers, assisted living facilities, wound care centers, and outpatient locations. This position focuses on the evaluation and management of peripheral arterial disease (PAD), chronic limb-threatening ischemia (CLTI), venous disease, diabetic foot ulcers, and non-healing wounds. The NP will work collaboratively with vascular specialists, wound care providers, primary care physicians, podiatrists, and nursing facility staff to identify patients requiring vascular intervention while helping prevent limb loss through early diagnosis and treatment.
