About The Position

Shelter House is a community of residents, staff and volunteers working together to provide basic human services for the men and women of the Greater Cincinnati area who are experiencing homelessness. The Case Manager is responsible for providing intensive case management services to individuals experiencing homelessness with a primary emphasis on housing, income benefits and prevention of a return to homelessness.

Requirements

  • Bachelors degree required
  • Valid driver’s license with good driving record, daily access to private insured vehicle during work hours and the ability to be approved by Shelter House insurer.
  • Must be able to work nights, weekends, and/or holidays.
  • Ability to function effectively in a high energy environment
  • Ability to work independently
  • Ability to work flexible hours including weekends and holidays
  • Time spent walking/standing- 50%
  • Time spent sitting- 50%
  • Works on computer 100% of time
  • Must be able to lift 20 pounds or more

Nice To Haves

  • LSW/MSW preferred
  • CDCA preferred
  • Experience with case management, homelessness, mental health or substance abuse

Responsibilities

  • Provide services to clients, including transportation
  • Assist clients with activities that promote achievement of their goals
  • Document client history, updates and goals in VESTA
  • Contact, or attempt contact, with clients once a week
  • Create housing plans for clients (ISP)
  • Complete case management process of intake and psychosocial assessment
  • Manage GPD vet dorms
  • Constant veteran outreach
  • Attend VA workgroups bi-weekly
  • On a monthly basis, perform outreach at Hamilton County Justice Center
  • Works with clients matched to the Rapid Rehousing Program. About 50% of time will be spent working with clients under this project.
