About The Position

Community Health Center, Inc. (CHC) is seeking a full-time Urgent Care Provider (APRN or PA) for its New Britain, CT location. CHC is a leading provider of primary medical, dental, and mental health services, dedicated to caring for special populations and improving health outcomes. This role involves delivering comprehensive primary and urgent care services within a patient-centered medical home model. The provider will assess and treat common acute and episodic illnesses in patients of all ages, provide preventative services, perform diagnostic testing, educate patients and families, utilize evidence-based standards for treatment, document care in an electronic health record, and participate in quality improvement initiatives.

Requirements

• Graduate of an accredited Master of Science in Nursing program or Physician Assistant Master's program.
• Licensed as either a Family Nurse Practitioner or Physician Assistant.

Responsibilities

• Assess and treat common acute and episodic illness in patients of all ages.
• Provide preventative services as appropriate.
• Perform appropriate diagnostic waived testing as required and arrange for follow up care with PCP as indicated.
• Provide problem-specific education to individual and family members as appropriate.
• Utilize evidence based standards in formulating treatment plan, and communicate rationale for decision making with patients.
• Document all findings, including appropriate plan for follow up care, in electronic health record.
• Participate in the organization’s quality improvement program.
