About The Position

Community Health Center, Inc. (CHC) is seeking a motivated and diverse professional to join their team as an Urgent Care Provider (NP or PA) in Bristol, CT. CHC is a leading provider of primary medical, dental, and mental health services, dedicated to caring for special populations and improving health outcomes. They are committed to building healthy communities and offer comprehensive primary care services across medicine, dentistry, and behavioral health. With a large patient base, CHC focuses on innovative service delivery and state-of-the-art technology within the Patient-Centered Medical Home Model (PCMH). This is a part-time, 22-hour position supporting the CHC location in Bristol, CT, focusing on delivering full primary care and urgent care services in a patient-centered medical facility. The ideal candidate will work two 10-hour days with an additional 2 hours of administrative time per week.

Requirements

• Graduate of an accredited Master of Science in Nursing program or Physician Assistant Master's program.
• Licensed as either a Family Nurse Practitioner or Physician Assistant.

Responsibilities

• Assess and treat common acute and episodic illness in patients of all ages.
• Provide preventative services as appropriate.
• Perform appropriate diagnostic waived testing as required and arrange for follow up care with PCP as indicated.
• Provide problem-specific education to individual and family members as appropriate.
• Utilize evidence based standards in formulating treatment plan, and communicate rationale for decision making with patients.
• Document all findings, including appropriate plan for follow up care, in electronic health record.
• Participate in the organization’s quality improvement program.
