About The Position

The Triage Nurse will join the Clinical Team at busy multi-cultural ambulatory care centers and help provide the most efficient quality clinical care to patients. This position is responsible for assessing patients and establishing the level of care that they require both virtually (chat, video, telephone) and in-person to the patient population of Delaware Valley Community Health, Inc in a safe and timely manner. Care can range from urgent/episodic to scheduled, preventative care and chronic disease management.

Requirements

  • Graduation from an accredited school of nursing required.
  • Current PA license as a Licensed Practical Nurse (LPN) or a Registered Nurse (RN).
  • CPR Certification required.
  • Demonstrated strong critical thinking, collaboration and communication skills.
  • Demonstrated ability to handle multiple in bound calls/multiple phone lines.
  • Demonstrated use of technology as a platform for patient care and connected clinical devices.
  • Must be able to present a positive and enthusiastic demeanor while directly working with patients and staff.

Nice To Haves

  • Preferred experience is medical/surgical and/or experience in critical or emergency care with specialized triage training.
  • Preferred experience in primary care, urgent care, emergency care, and/or ambulatory care.
  • Bilingual Spanish/English preferred.

Responsibilities

  • Interviews/triages patients and or family to determine the need for medical attention.
  • Collects and interprets patient data in consultation with a provider regarding appropriate management and treatment for the initial evaluation of a problem or the follow-up of a previously diagnosed condition.
  • Records all patient data in patient electronic health record (EHR) after each encounter.
  • Initiates diagnostic and therapeutic measures as indicated by standing protocols.
  • Provides health counselling and instructions regarding common patient problems.
  • Works with Clinical Pharmacist in supporting patient education regarding medications and adherence to medications and appointments.
