About The Position

RiverView Health, a community-owned, membership-based non-profit organization established in 1898, is seeking a Treatment Coordinator. This role is crucial for providing non-clinical coordination to individuals with substance use disorders and co-occurring conditions. The focus is on service linkage, resource navigation, care continuity, and ensuring documentation compliance with MN Statutes. The position may also involve supporting ancillary services that aid clients in maintaining stability and daily functioning. RiverView Health operates a 25-bed Critical Access Hospital, a chemical dependency outpatient treatment program (RiverView Recovery Center), RiverView Home Care, and five primary care and specialty clinics.

Requirements

  • High school diploma or equivalent.
  • Enroll and complete 15 hours Treatment Coordinator education or training prior to or within 60 days of hire.
  • Must be 18 years of age or older.
  • Ability to pass Minnesota Department of Human Services criminal background study.
  • Adherence to documentation & Compliance to MN Statute 245G and agency policies.
  • Ensure accurate data entry for patient registration and scheduling.
  • Ability to work effectively in a team environment.
  • Good communication skills (verbal & written), organization, leadership, and problem-solving.

Nice To Haves

  • Two years of full-time experience providing direct services to individuals with substance use disorders and/or mental health conditions.
  • CPR/First Aid Certification.

Responsibilities

  • Service Coordination & Linkage: Assess client needs and facilitate connections to medical, mental health, housing, employment, and recovery services. Monitor follow-through to ensure successful engagement.
  • Resource Navigation & Barrier Reduction: Guide clients through insurance, benefits, and service systems. Problem-solve logistical barriers including transportation and scheduling.
  • Care Continuity & Communication: Facilitate communication between clients, families, clinical staff, and external providers to support transitions across levels of care.
  • Administrative Support: Provide scheduling, reception, appointment reminders, and office functions essential to daily program operations.
  • Collaboration: Participate in case reviews, clinical supervision, and quality improvement initiatives.
  • Ancillary Support: May assist with daily living skills, life skills development, routine stabilization, community integration, and risk identification to support client stability, based on training and role development.
