Requirements

High school diploma or equivalent.
Enroll and complete 15 hours Treatment Coordinator education or training prior to or within 60 days of hire.
Must be 18 years of age or older.
Ability to pass Minnesota Department of Human Services criminal background study.
Adherence to documentation & Compliance to MN Statute 245G and agency policies.
Accurate data entry for patient registration and scheduling.
Ability to work effectively in a team environment.
Good communication skills (verbal & written), organization, leadership, and problem-solving.

Nice To Haves

Two years of full-time experience providing direct services to individuals with substance use disorders and/or mental health conditions.
CPR/First Aid Certification.

Responsibilities

Provide non-clinical coordination for individuals with substance use disorders and co-occurring conditions, focusing on service linkage, resource navigation, care continuity, and documentation in compliance with MN Statutes.
Assess client needs and facilitate connections to medical, mental health, housing, employment, and recovery services, monitoring follow-through to ensure successful engagement.
Guide clients through insurance, benefits, and service systems, and problem-solve logistical barriers including transportation and scheduling.
Facilitate communication between clients, families, clinical staff, and external providers to support transitions across levels of care.
Provide scheduling, reception, appointment reminders, and office functions essential to daily program operations.
Participate in case reviews, clinical supervision, and quality improvement initiatives.
May assist with daily living skills, life skills development, routine stabilization, community integration, and risk identification to support client stability, based on training and role development.
