Requirements

  • High school diploma or equivalent.
  • Enroll and complete 15 hours Treatment Coordinator education or training prior to or within 60 days of hire.
  • Must be 18 years of age or older.
  • Ability to pass Minnesota Department of Human Services criminal background study.
  • Adherence to documentation & Compliance to MN Statute 245G and agency policies.
  • Accurate data entry for patient registration and scheduling.
  • Ability to work effectively in a team environment.
  • Good communication skills (verbal & written), organization, leadership, and problem-solving.

Nice To Haves

  • Two years of full-time experience providing direct services to individuals with substance use disorders and/or mental health conditions.
  • CPR/First Aid Certification.

Responsibilities

  • Provide non-clinical coordination for individuals with substance use disorders and co-occurring conditions, focusing on service linkage, resource navigation, care continuity, and documentation in compliance with MN Statutes.
  • Assess client needs and facilitate connections to medical, mental health, housing, employment, and recovery services, monitoring follow-through to ensure successful engagement.
  • Guide clients through insurance, benefits, and service systems, and problem-solve logistical barriers including transportation and scheduling.
  • Facilitate communication between clients, families, clinical staff, and external providers to support transitions across levels of care.
  • Provide scheduling, reception, appointment reminders, and office functions essential to daily program operations.
  • Participate in case reviews, clinical supervision, and quality improvement initiatives.
  • May assist with daily living skills, life skills development, routine stabilization, community integration, and risk identification to support client stability, based on training and role development.
