About The Position
Rural hospital located several hundred miles northwest of Anchorage, AK seeking a full-time, Travel Social Worker to join their team. This is a 6-month contract opportunity. The role involves providing medical social work services to patients of all ages, including those in the Acute Care Unit and Primary Care Clinic. Responsibilities include case management, discharge planning, coordinating nursing home/assisted living placements, risk management, crisis intervention, and care coordination.
Requirements
- Must be a licensed Social Worker
- Performing medical social work, child protection or adult protection services or related field
- Ability to use and implement the psycho-social diagnostic process of assessment, diagnosis, planning, intervention and evaluation
Responsibilities
- Provide medical social work services to patients of all ages, including the patients located in the Acute Care Unit, Primary Care Clinic
- Respond to requests for medical social work services, including case management, discharge planning, nursing home/assisted living placements, risk man agreement, crisis intervention, and care coordination
- Coordinate nursing home and assisted living placements for patients.
