Travel Social Worker
About The Position
A rural hospital several hundred miles northwest of Anchorage, Alaska, is seeking a full-time Travel Social Worker. The role provides medical social work services to patients of all ages, including patients in the Acute Care Unit and Primary Care Clinic. The position is a six-month contract. Pay is $60.00 per hour, and housing and flights are provided. The position is based in a rural, remote Alaskan town. Requirements
- Must be a licensed Social Worker.
- Experience performing medical social work, child protection or adult protection services, or work in a related field.
- Ability to use and implement the psychosocial diagnostic process of assessment, diagnosis, planning, intervention, and evaluation.
Responsibilities
- Provide medical social work services to patients of all ages, including patients in the Acute Care Unit and Primary Care Clinic.
- Respond to requests for medical social work services, including case management, discharge planning, nursing home and assisted living placements, risk man agreement, crisis intervention, and care coordination.
- Coordinate nursing home and assisted living placements for patients.
- Use and implement the psychosocial diagnostic process of assessment, diagnosis, planning, intervention, and evaluation.
