About The Position

The Travel Physical Therapist provides skilled physical therapy in patients’ homes to restore, maintain, and promote mobility, independence, and quality of life. This role serves adults and geriatric patients with post-surgical, acute, or chronic conditions who require mobility, balance, or functional rehabilitation. Assignments are typically 13 weeks and require flexibility for multiple patients or float coverage, with potential for extensions based on performance.

Requirements

• Graduate of an accredited Physical Therapy program
• Active PT license in assigned state(s)
• CPR/BLS certification
• Minimum one (1) year of clinical experience preferred
• Travel flexibility and ability to adapt to home environments

Responsibilities

• Evaluate patient mobility, strength, and functional status
• Develop and implement individualized care plans
• Provide therapy interventions (exercise, gait training, functional mobility)
• Educate patients and caregivers on exercises, safety, and equipment
• Recommend and train in use of durable medical equipment (DME)
• Complete accurate and timely documentation per client and regulatory standards
• Supervise Physical Therapist Assistants if applicable
• Meet client-established visit and documentation targets
• Maintain high-quality care while meeting productivity standards
• Participate in audits and performance improvement initiatives
