About The Position
Navitas Healthcare, LLC is seeking a Licensed Practical Nurse (LPN/LVN) to provide direct patient care in a long-term care setting. The role involves medication administration, monitoring resident health, and supporting patients with chronic conditions including Alzheimer’s and dementia.
Requirements
- Active New York LPN/LVN License (Required)
- Minimum 1 year of experience
- BLS Certification
- COVID-19 Vaccination (Required)
- Ability to complete compliance within 1.5 weeks
- Must commit to assigned shift
Nice To Haves
- LTC experience
- Experience with dementia/Alzheimer’s care
- Familiarity with PointClickCare EMR
- Background in rehab, med-surg, or hospital settings
- First-time travelers welcome
Responsibilities
- Administer medications safely (Med Pass)
- Monitor patient condition and report changes
- Provide care for dementia and Alzheimer’s patients
- Assist with daily living activities (ADLs)
- Maintain accurate documentation
- Follow infection control and facility protocols
- Collaborate with interdisciplinary care team
