The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. This role demonstrates knowledge of nursing principles for the resident in the relevant clinical setting and practices within the full scope of nursing expertise/knowledge. The LPN utilizes appropriate age and population specific standards related to the physical and psychosocial needs of the resident as per care plan. The position requires the ability to perform testing to evaluate data reflective of the resident's status and to provide the care needed. The LPN must be able to recognize problems, recommend solutions and respond effectively to assist in an emergent situation, ensuring the health, comfort and safety of residents. The role contributes to the assessment of residents and administers medications or treatments as ordered, and provides technical support to healthcare professionals as needed. Documentation of resident interactions and their outcomes related to resident care, medication, behavior, activity, and dietary is essential. The LPN must be skilled in establishing and maintaining effective working relationships with residents, health care providers, and the public, and be able to work effectively with professional and supportive personnel as a member of an interdisciplinary care team. The ability to communicate clearly and maintain confidentiality of sensitive information is required. The LPN will counsel residents, family members and/or resident advocates, under the direction of a registered nurse, advanced practice provider or physician on prevention and treatment plans, and possess knowledge of resident education principles to develop a self-care program.
