About The Position

Requirements
- Active LPN license required
- Strong medication administration and patient assessment skills
- Ability to work effectively in a fast-paced healthcare environment
- Excellent communication, teamwork, and organizational skills
Nice To Haves
- Previous long-term care or skilled nursing experience preferred
Responsibilities
- Provide direct nursing care to long-term care and rehabilitation residents
- Administer medications and treatments accurately and safely
- Monitor patient conditions and report changes promptly to supervising staff
- Maintain accurate nursing documentation and charting
- Assist with admissions, transfers, and discharge processes
- Collaborate with CNAs, nurses, physicians, and interdisciplinary care teams
- Support infection control and resident safety standards
- Respond effectively to emergencies and patient care needs
- Promote compassionate and respectful resident-centered care
