Role responsibilities and growth: As an LPN in Long-Term Care, you’ll provide direct patient care, monitor health status, administer medications within scope, assist with activities of daily living, help with mobility and nutrition, manage wounds and skin care, and support infection control. You’ll participate in care planning with RNs and the interdisciplinary team, document accurately in electronic health records, and communicate changes to the team and families with clarity and empathy. You’ll support falls prevention, discharge planning, and end-of-life care as appropriate.
