About The Position

This position involves assisting in the delivery of patient care under the supervision of a Registered Nurse (RN) in a long-term care setting. The Licensed Practical Nurse (LPN) will provide age-specific nursing care, guide resident care activities while maintaining nursing standards, and monitor patient conditions. Responsibilities include maintaining accurate patient records, communicating with physicians and families, and directing nurse assistants and aides. The role requires a commitment to confidentiality and fostering a diverse, inclusive, and equitable working environment.

Requirements

  • Diploma from an accredited School of Nursing.
  • Certification by the state as a Licensed Practical Nurse Required.
  • Current BLS required within four (4) months of hire or transfer.
  • Kind and professional demeanor.
  • Professional and well-groomed appearance, including during on call hours.
  • Communicate positively and effectively, both written and verbally, with patients, family, and staff.
  • Demonstrate effective organizational skills in an evolving environment.
  • Work with honesty, compassion and integrity at all times.
  • Adherence to ALL PMC confidentiality procedures and policies.
  • Demonstrate a commitment to building and sustaining a diverse, inclusive, and equitable working environment.
  • Must be able to remain in a stationary position 50% of the time.
  • Must be able to move and traverse about the facility 50% of the time.
  • Frequently transport objects weighing up to 50lbs.
  • Occasionally position objects weighing up to 100lbs.
  • Must be able to communicate and exchange information in a way others will understand.
  • Must be able to recognize details such as color and depth within a few feet of the observer.
  • Frequently operates computers, machinery, and other healthcare equipment.
  • Constantly positions self to complete essential functions.
  • May be required to wear N95s or PAPRs up to 80% of the shift.

Nice To Haves

  • At least six (6) months experience working as a CNA preferred.

Responsibilities

  • Assist in the delivery of patient care under the supervision of an RN.
  • Provide age-specific nursing care under the supervision of an RN.
  • Guide resident care activities while maintaining standards of nursing practice.
  • Monitor patient conditions during treatments, procedures, and activities.
  • Maintain accurate and detailed records of patient care, medical histories, and treatment plans.
  • Read and interpret patient information, making treatment decisions when appropriate.
  • Contact Physician and resident's families when appropriate and necessary.
  • Direct and supervise nurse assistants and nurse aides.
  • Other duties as assigned.
