About The Position

The Transportation Coordinator is responsible for coordinating and dispatching transportation routes, including both in-house and outside appointments. This role also involves participating in Inter Disciplinary Team (IDT) and Care Planning meetings. The company's mission is to empower senior participants to age at home with dignity through personalized, comprehensive care plans that deliver high-quality health and human services along with strong community support.

Requirements

  • Minimum of one (1) year of documented experience in working with a frail or elderly population.
  • Must have a valid California Driver’s License.
  • Is medically cleared for communicable diseases and has all immunizations up-to-date before engaging in direct participant contact.
  • Knowledge of the area in which Innovative Integrated Health is located.
  • Ability to cope with mental and emotional stress related to the position.
  • Function independently, have flexibility, and personal integrity.
  • Must demonstrate on an ongoing basis the ability to develop and maintain good working relationships with co-workers and department heads.
  • Effective written and oral communication skills.
  • Ability to access all areas of the center throughout the workday is required.
  • Ability to communicate verbally and in writing with an excellent comprehension of the English language is required.

Responsibilities

  • Participate as a member of the Inter Disciplinary Team and Care Planning meetings, and report changes in participants’ baseline status to appropriate staff.
  • Collaborate with applicable departments to coordinate transportation for all participants scheduled clinic and outside provider appointments, day center and outings.
  • Involved in the implementation of Quality Improvement activities.
  • Develop and maintain transportation routes manually and with transportation software.
  • Dispatch and communicate new routes to drivers with their participant routes.
  • Prepare transportation metrics and driver performance and safety reporting for Management, as needed.
  • Prepare monthly transportation ride count reporting.
  • Work with Transportation Assistant(s) and Drivers to ensure vehicle maintenance and repairs.
  • Work with other departments to coordinate additional routes, meal delivery, DME and/or care packages.
  • Attend and participate in staff meetings, in-services, projects, and committees as assigned.
  • May be asked to drive a company vehicle and complete routes to provide support for the department.
  • Understand and navigate the Electronic Health Records system.
  • Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance.
  • Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.
  • Be flexible in the schedule of hours worked.
  • May require use of personal vehicle.
