About The Position

ChristianaCare's Transplant Program is seeking a dedicated and collaborative Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive care to transplant patients in inpatient and outpatient settings. The clinician will function as an independent member of a multidisciplinary team and collaborate with transplant surgeons and other specialized healthcare professionals throughout the transplantation and living donation journey. This role is based at the Newark Campus and has daytime hours Monday through Friday, 7:30 a.m. to 4:00 p.m., with occasional on-call and weekend responsibilities.

Requirements

Master's degree as an Acute Care Nurse Practitioner or completion of an accredited Physician Assistant program.
License or licensure as an APRN or Physician Assistant in the state of Delaware.
BLS and ACLS certification.
Two years of experience as an Advanced Practice Clinician in a surgical setting.

Responsibilities

Apply surgical expertise while collaborating with transplant surgeons and the multidisciplinary transplant care team to coordinate care throughout transplantation and living donation.
Perform comprehensive patient assessments, obtain histories, conduct physical examinations, and develop individualized care plans.
Perform procedures within approved clinical privileges.
Manage transplant patients in inpatient and outpatient settings.
Provide care for post-operative transplant patients and patients requiring medical readmission.
Participate in daily rounds independently and with transplant surgeons and multidisciplinary team members.
