About The Position

The Pediatric Transplant Specialist serves as a key coordinator and first point of contact for patients referred to SCH for pediatric transplant evaluation. This role is responsible for managing the intake, tracking, and coordination of complex transplant referrals, ensuring that all required clinical documentation, authorizations, and regulatory elements are complete, accurate, and timely. Working within a multidisciplinary pediatric transplant team, the Transplant Specialist partners closely with transplant physicians, advanced practice providers, nurses, social work, finance, and external referring providers to support seamless referral workflows. This role requires a high level of attention to detail, robust organizational skills, and the ability to communicate with empathy and professionalism while supporting children and families navigating emotionally complex and high-acuity care. The Pediatric Transplant Specialist plays an important role in supporting regulatory compliance, maintaining referral data and audit-ready documentation, monitoring referral timelines, and identifying and escalating barriers to progression. Through proactive coordination and a family-centered approach, this role helps ensure safe, equitable, and efficient access to pediatric transplant services while supporting operational excellence and an exceptional patient and family experience.

Requirements

  • High school diploma or equivalent experience.
  • Minimum of two (2) years of healthcare experience.
  • Demonstrated experience coordinating across multiple disciplines (providers, RNs, APPs, social work, finance, insurance, etc.)

Nice To Haves

  • Preferred Bachelor’s Degree or at least two (2) years of college coursework in health care or related field.
  • Familiarity with transplant-specific regulations (CMS, OPTN/UNOS)
  • Knowledge and experience with healthcare payor authorizations.
  • Lean, Six Sigma, or process-improvement training
  • Familiarity with other cultures/bilingual skills.
