About The Position

Mercy Housing is one of the nation's largest affordable housing nonprofit organizations. We create and preserve affordable housing and provide supportive resident services that help families, seniors, veterans, and individuals build stable, healthy futures. The Healthcare Coordination Specialist will develop strategies to link residents with health care and care coordination services. You will build relationships with primary care clinics, medical providers, care coordinators, health plans and other health service providers to increase the health and wellness of residents and reduce unnecessary healthcare utilization. This is a grant-based position that will be performed onsite.

Requirements

  • High School Diploma or equivalent.
  • Two (2) years of experience in a position that involved coordination among and frequent interaction with patients/clients, health care providers, care coordinators, health plans, and other service systems.

  • Bachelor’s Degree in Healthcare Management, Public Health, or related field.

Responsibilities

  • Maintain detailed knowledge of the healthcare system and key service providers.
  • Collaborate with Resident Service Coordinators (RSC) to identify residents most in need of care coordination and self-management assistance and determine what staff member is most appropriate to provide this assistance.
  • Accurately identify/assess resident assets and needs in order to connect them effectively to resources.
  • Develop partnerships with primary care clinics, medical providers, hospital discharge planning team, care coordinators, health plans and other health service providers who work with the largest numbers of residents at high risk of health problems and elevated health care utilization (“high health risk”).
  • Implement procedures for consented exchange of information to facilitate collaborative care coordination.
  • Work one-on-one with selected high-risk residents to increase their engagement with health care, care coordination and wellness services.
  • Coordinate with RSCs, prioritize and develop onsite health education and prevention services for residents.
  • Develop additional strategies to link residents with health and care coordination services.
  • Collect required data for designated program components and record data in the Mercy Housing database in an accurate and timely way.
  • Actively collaborate with Resource Development and Resident Services to share information and implement fundraising plans.
