About The Position

This position provides personal care services or accommodations to clients enrolled in the Transition to Independence Process (TIP) program. This can include planning and leading skill development groups, assisting with meal preparation, supervising the self-administration of medication and assisting with activities of daily living. This position is full-time and second shift, working from 2:45pm-11:00pm and has a rotating 2-week schedule. For the first week, work days are Monday, Tuesday, Thursday and Saturday. For the second week, work days are Sunday, Monday, Tuesday, Wednesday and Friday with a mandatory staff meeting every Thursday from 3:00pm-4:30pm.

Requirements

  • High School Diploma or GED required.
  • CPR/First Aid Certification required before working alone.
  • NCI/CPI Certifications required within 90 days of employment.
  • Test for tuberculosis before hire or provide evidence of negative test within one year prior to employment.
  • Must have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier.

Nice To Haves

  • Experience working with clients diagnosed with severe and persistent mental illness.
  • Ability to work a flexible schedule is preferred.

Responsibilities

  • Supervises and/or assists residents with activities of daily living.
  • Supervises the self-administration of medication.
  • Assists or prepares resident meals.
  • Develops the individualized care plan for residents and provides care in accordance with that care plan. Ensures the House Manager has any specific information or observations of the residents’ needs and preferences to pass along to the clinical team.
  • Completes activities with residents to support their transition to independent living including weekly goal planning, connecting with resources in the community, and pursuing vocational and/or educational goals.
  • Maintains accurate, complete, and appropriate clinical records (e.g., MAR, daily resident logs) in a timely manner.
  • Performs all job responsibilities in accordance with prescribed safety and infection control procedures.
  • Implements planned recreational/educational activities.
  • Transports residents as needed for appointments or other approved activities as determined by job assignment and in accordance with driving policies.
  • Responsible for the completion of all tasks for assigned shift including care of residents and the facility, which may include cleaning and minor household maintenance.
  • Responsible for finding coverage for scheduled shifts that employee is unable to work due to illness, vacation, or other time off. Ability to work a flexible schedule is preferred.
  • Participates in training and development programs to enhance knowledge and skills and completes annual trainings as assigned, including trainings on the TIP model and its implementation.
  • Maintains courtesy and professionalism with staff and clients at all times.
  • Understands and maintains professional boundaries in all client and staff interactions.
  • May be asked to float to other housing locations to accommodate client or agency needs.
  • Required to participate in the on-call team rotation by taking at least two weeks per quarter. On-call rotation includes being the first individual called by on-shift staff for questions, concerns, and finding coverage, as well as providing shift coverage when other options are not available.
