Transitional Healthcare Facilitator - Re-Entry Facilitator
About The Position
Centurion is seeking a full-time Transitional Healthcare Facilitator (also known as a Re-Entry Coordinator) for needs at Rockville Correctional Facility in Rockville, Indiana. The Transitional Healthcare Facilitator will be responsible for the development and implementation of transitional healthcare plans for incarcerated individuals scheduled for release. The Transitional Healthcare Facilitator will coordinate with the Indiana Department of Correction and other state and local agencies to ensure that pre-discharge planning and preparation is accomplished, and those re-entering society are provided with necessary services and resources.
Requirements
- Bachelor’s degree in psychology, social work, counseling or related field from an accredited institution required
- Experience with/knowledge of community and state programs and services
- Must be able to pass background investigation and obtain agency security clearance where applicable
Nice To Haves
- Two (2) years of experience in re-entry services or release planning preferred
Responsibilities
