About The Position

This role focuses on coordinating transitions for individuals with intellectual and developmental disabilities (IDD), particularly those with behavioral/mental health concerns, criminal/forensic involvement, and complex needs. The Transitional Coordinator will work closely with various agencies and professionals to develop and implement individualized plans, facilitate community integration, and provide ongoing support during and after transitions.

Requirements

  • A Master’s Degree in Psychology, Social Work, Counseling.
  • Five years of experience in the past 7 years working with people with IDD with behavioral/mental health concerns with criminal and/or forensic involvement.
  • Extensive experience in the development and implementation of Individual Program Plans and person-centered plans for people with IDD with criminal and forensic involvement.
  • Extensive experience in linking people with IDD with complex behavioral/mental health and forensic needs to community supports and resources.

Nice To Haves

  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC) credential

Responsibilities

  • Collaborate with DDS and the Regional Centers on the Intake and Referral Process.
  • Facilitate and lead Intake/admission meetings.
  • Collaborate with the Clinical Coordinator, Consultants, and other members of the IDT in the development and implementation of the IPP and Person Centered Plan and transition plans.
  • Assist in conducting Risk Assessments in collaboration with the Director and Clinical Director.
  • Coordinate with Regional Center Liaison and Service Coordinator on accessing services in the community as per IPP and Transition Plan.
  • Support the person pre transition, during transition and post transition with at a minimum of monthly visits or more frequently as necessary.
  • Performs all other duties, as assigned.
