About The Position

SLCS is currently seeking a Full-time Transitional Case Worker for Gallagher Centre and Maison Louise Arbour (MLA). As a member of this interdisciplinary team you will provide referral and intake management, crisis intervention, discharge planning and support services for clients who may be involved in the criminal justice system and/or struggle with mental health and addiction related issues. You will be expected to assess clients’ risk, skills and needs, and collaborate with clients to form Individualized Support Plans that support their overall wellness and community reintegration. Within this role, you will collaborate with other professionals such as but not limited to: probation officers, court workers, lawyers, correctional staff, and any other community supports to assist clients with their reintegration and wellness goals. This role is heavily focused on short term transitional services. You will complete case management duties that include daily check ins with clients, completion of monthly Individualized Support Plans, case noting, participation in monthly Case Reviews, and the navigation of applicable resources within the community. You will also assist in tracking client statistics, as well as manage referrals and intakes for the program. In addition to the clinical elements of the position, you will work collaboratively with other team members at Gallagher and MLA to support the safety and security of staff and clients. You will also support staff with administration duties when necessary. You will be skilled at working with clients from diverse populations; contributing to program development; as well as providing education support and supervision to students. You will have a strong understanding of risk and be skilled in assessment and intake practices. This position is primarily located at Gallagher Centre but will include regular travel between SLCS locations and others within the community. This position is working from 8am - 4pm, Monday - Friday.

Requirements

Post-secondary education from an accredited institution
Minimum of 1-year relevant experience preferably within justice, corrections, mental health and/or addictions
Standard First Aid / CPR with AED – Level C
Valid G Class Driver’s License and a reliable vehicle
Clear Police Check with vulnerable sector screening

Responsibilities
