The Addiction Transitional Case Manager will perform duties and responsibilities consistent with providing a psychosocial model of care to assist in streamlining access to community addiction and mental health supports for individuals who are presenting to hospital services. This is a front-line, clinical position providing crisis intervention, brief addiction transitional case management, and system navigation related to substance use health. This position provides support and care pathways to community-based addiction and mental health services in a collaborative manner to reduce avoidable, unnecessary and repeat emergency department visits or admissions. Substance iNtervention Adolescent Program (SNAP) Staff will support the ATCM program as required and will receive referrals and support the SNAP program which consists of a multidisciplinary team. This team includes Nurse Practitioner’s, Physician’s, Nurses, Orderly’s, Clerks, Social Workers and Peer Harm Reduction Specialists from London Health Science Centre or other partnering agencies. The ATCM will work primarily within the inpatient and outpatient pediatric programs supporting the SNAP team with the purpose of supporting patients with Substance Use Health concerns after receiving a request for consultation for identified Substance Use Health concerns within London Health Sciences Centre.
