About The Position

In collaboration with the patient and family, physicians, and the interdisciplinary team, the Transitional Care Nurse Navigator facilitates the patient’s progress through an acute episode of care in an efficient and cost-effective manner. The role creates a transition plan with the patient, family, and providers for moving from the inpatient setting to a post-acute setting. The nurse navigator initiates evidence-based protocols and order sets according to the patient’s admission diagnosis, influences the progression of care, and facilitates navigation through the acute care episode. The role proposes an appropriate transition plan within 48 hours of admission, advocates for the patient and family throughout the care continuum, and facilitates communication with the multidisciplinary team, referring physician, and ambulatory care team. The nurse navigator also fosters patient and family self-management and adherence to the individualized plan of care through a post-acute phone call.
