About The Position

Horizon Home Health and Hospice is seeking a Transitional Care Coordinator to establish and maintain positive relationships with customers and referral sources, and to respond to customer requests and concerns. Requirements

  • Establish and maintain positive working relationships with current and potential referral and payer sources.
  • Build and monitor community, customer, payer, and patient perceptions of Horizon Home Health and Hospice as a high-quality provider of services.
  • Maintain comprehensive working knowledge of Horizon Home Health and Hospice markets, including government agencies, major payer groups, key referral sources, and competitor's market positioning.
  • Maintain comprehensive working knowledge in the field of marketing and share information with appropriate organization personnel.
  • Maintain comprehensive working knowledge of community resources and assist customers in accessing community resources if services are not provided by Horizon Home Health and Hospice.

Responsibilities

  • Works with the Clinical Supervisor to establish community education for clinical programs.

  • Maintains comprehensive working knowledge of community resources and assists customers in accessing community resources should services not be provided by Horizon Home Health and Hospice.
