  • Home Health and Hospice Intake experience

Responsibilities

  • Utilize computerized records program to process, store, and retrieve health information.
  • Analyze and review medical records for completeness and accuracy of documentation according to specified standards.
  • Ensure agency compliance with medical record policies, procedures, and documentation requirements of federal and state regulatory agencies.
  • Maintain records related to HIPAA compliance and assure agency compliance with the Health Information Portability and Accountability Act.
  • Maintain clinical records by creating files, organizing, and maintaining records to meet standards of practice.
  • Confer with doctors, nurses, and other health personnel to ensure complete, current, and accurate medical records.
  • File documentation accurately in patient records and keep filing current.
  • Audit records, identify inaccuracies or delinquencies, and keep management updated on trends.
  • Follow up with appropriate individuals to resolve incomplete records.
  • Provide staff training on medical record requirements.
  • Retain old records in accordance with medical record retention requirements and arrange for file destruction when appropriate.
  • Work as part of the branch office team, assist with handling phone calls, attend team meetings, and support other functions as needed.
  • Schedule and appropriately document patient schedules in a timely manner.
  • Participate in coordinating care with management and the patient interdisciplinary team.
  • Provide effective communication to patients, staff members, other healthcare professionals, and referral sources.
  • Respond to agency and patient needs in a professional and creative manner.
  • Demonstrate commitment, professional growth, and competency.
  • Promote Agency philosophy and administrative policies to ensure quality of care.
  • Perform other duties as assigned.
