About The Position
Horizon Home Health & Hospice is seeking a full-time, salaried Physical Therapist Case Manager to join our home-health team in Caldwell, Idaho. This role is ideal for PTs who value autonomy, meaningful patient relationships, and the opportunity to deliver high-quality care directly in patients’ homes. As a PT Case Manager, you will oversee a balanced caseload, provide skilled therapy interventions, coordinate interdisciplinary services, and guide patients through recovery, functional improvement, and safe transitions of care. Requirements
- Current Idaho Physical Therapist license (or ability to obtain).
- One year of PT experience preferred; home-health experience a plus.
- Strong communication, assessment, and clinical reasoning skills.
- Ability to work independently in a field-based environment.
- Reliable transportation and valid driver’s license.

Responsibilities
- Perform comprehensive physical therapy evaluations in patient homes.
- Develop individualized treatment plans focused on functional mobility, safety, and independence.
- Provide therapeutic interventions including gait training, strengthening, balance work, and home-exercise program development.
- Conduct home-safety assessments and recommend appropriate DME or environmental modifications.
- Manage a field-based caseload with strong organizational efficiency.
- Coordinate care with interdisciplinary team members to ensure seamless service delivery.
- Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
- Communicate effectively with physicians, caregivers, and internal team members.
- Support safe transitions from hospitals, SNFs, and outpatient settings.
