About The Position

The Arc of Ocean County is seeking a Transition Services Specialist to help adults with intellectual and developmental disabilities build the skills, confidence, and experience they need to pursue employment, increase independence, and engage more fully in their communities. In this role, you'll combine individualized instruction, hands-on coaching, and community-based experiences to help participants work toward their personal and employment goals. From developing workplace skills and exploring career opportunities to supporting community involvement and independent living, your guidance can help turn small accomplishments into meaningful progress.

Requirements

  • Bachelor's degree in Human Services, Psychology, Social Work, Rehabilitation Services, Special Education, or a closely related field.
  • Valid New Jersey driver's license with a satisfactory driving record (five points or fewer).
  • Excellent communication, organizational, and relationship-building skills.
  • Ability to work independently while collaborating effectively with coworkers, families, employers, and community partners.
  • Flexibility to participate in community-based programming throughout Ocean County.
  • Ability to lift and carry at least 25 pounds when necessary.
  • Ability to assist individuals with mobility-related needs.
  • Ability to travel throughout the community and actively participate in off-site programming.

Responsibilities

  • Provide instruction focused on employment preparation, communication, independent living, and interpersonal development.
  • Create activities and learning experiences that build confidence, accountability, and a sense of accomplishment.
  • Adjust instruction and teaching approaches according to each individual's abilities, learning style, and support requirements.
  • Encourage participants to develop self-advocacy skills, make choices, and work toward greater independence.
  • Guide participants through volunteer placements, career exploration, and other community-based learning experiences.
  • Help individuals practice professional behavior, workplace expectations, and communication skills in real-world settings.
  • Establish productive relationships with employers, businesses, and community organizations to help create meaningful opportunities.
  • Transport participants safely in agency vehicles to program locations and community activities.
  • Help participants pursue objectives established through their Individual Service Plans (ISPs).
  • Contribute to interdisciplinary team meetings by sharing relevant observations and updates on individual progress.
  • Complete required documentation accurately and within established timeframes, including daily service records, progress notes, medication administration records, and incident reports.
  • Apply positive behavior support approaches and respond appropriately to changing individual needs.
  • Administer medications according to agency policy and applicable regulations.
  • Follow individualized health, behavioral, nutritional, and adaptive support plans.
  • Promote dignity, respect, inclusion, and personal choice throughout all services and interactions.
  • Adhere to agency standards, safety practices, and program requirements.
