About The Position

Reporting to the Clinical Manager, the Therapy Support Navigator collaborates with all CASA programs and external partners to deliver an extension of mental health services and supports to children and adolescents with mental health concerns and their families. The Therapy Support Navigator is an active member of an interprofessional team involved in triage, intake, assessment, case management, psycho-educational/treatments groups, outreach services, resource navigation and transition services.

Requirements

  • Completion of any of the following: Bachelor of social work, Bachelor of arts, majoring in psychology, Bachelor of education or bachelor’s degree in a related field.
  • Relevant and practical experience in a child and adolescent mental health treatment setting.
  • Valid Class 5 operator’s license and access to transportation.
  • Adaptability and flexibility in fluid contexts and situations.
  • Strong knowledge and skill with behaviour management.
  • Ability to work independently and prioritize tasks appropriately.
  • Excellent organizational, interpersonal and communication skills.
  • A satisfactory Criminal Records Check with Vulnerable Sector Search.
  • A satisfactory Child Intervention Check.
  • Proof of COVID-19 vaccination or exemption on the basis of a protected legal ground.

Responsibilities

  • Actively engage in the triage process to determine the appropriate mental health services for patients and their families.
  • Assist Mental Health Therapists/Physicians in screening and assessment interviews to inform treatment plans.
  • Manage assigned caseload.
  • Provide hands-on modelling and coaching to family members, guided by the treatment plan.
  • Coordinate and facilitate psycho-educational/treatment groups for patients and their families.
  • Develop and maintain a list of community resources to refer patients and families to, for additional mental health support.
  • Actively engage in the Triage, Navigation and Transition process to determine the appropriate mental health services for patients and their families.
  • Engage in facility, school and/or home observations and document observation reports to inform treatment plans.
  • Conduct phone calls on behalf of clinicians (e.g. to schools, parents, other professionals) to gather information or explain strategies to assist the patient to be successful.
  • Complete relevant electronic medical record documentation.
