The Recovery Support Navigator is responsible for providing quality services and case management to assigned patients in accordance with the Recovery Navigator model. This role involves performing intake assessments on all new patients, developing and implementing individualized plans with goals and objectives based on identified needs, and assisting patients with scheduling and coordinating medical and behavioral health appointments. The Navigator will also coordinate care and plans with existing providers, including behavioral health, primary care, and Community Partners.
